NOTICE: As of January 1, 2014, WCG has enacted a policy change regarding patient payment. Patient's CO-PAYS and DEDUCTIBLE must be paid prior to examination. Payment for additional procedures ordered by doctors during the visit must be paid on the day services were rendered. All CO-PAYS and DEDUCTIBLE charges are estimates at best. The amount charged may be different from insurance company charges on the Explanation of Benefits (EOB). In the event of a discrepancy, our office will follow-up with any adjustments necessary. Thank you for your cooperation.

1. Making an Appointment
  • To schedule an appointment, please call 213-387-8000.
  • We have English, Korean, Spanish, Mandarin, Russian, and Tagalog speaking staff available.
  • Please expect the first visit to take at least 2 hours.

  • • Driver's License or Photo ID
  • • Current Insurance Card
  • • Current Medication List or All Prescribed Bottles
  • • Recent Lab Work/Diagnostic Procedures (if appropriate)
  • • Recent Hospitalization Records (if appropriate)
  • • FOR HMO PATIENTS: PLEASE BRING YOUR AUTHORIZATION GIVEN BY PRIMARY CARE PROVIDER
